A PERFECT NOSE

NoseA nose is the central feature on one’s face & defines facial beauty. Not everyone has a perfect nose, many people are born with a deformed nose. Rhinoplasty, also referred as ‘The Nose Job’ can fix the deformities. It is a surgery which needs a lot of finesse. After all the success & failure of a Nose Job is measured in millimeters. It used to be the province of celebrities & the affluent, however today it has become accessible to all & is one of the most popular aesthetic correction procedures performed globally & even in India. While I live & practice in Dubai, I travel around India to perform select procedures on requests. Over 60% of the surgeries I perform are Rhinoplasty. While a popular procedure, a Nose Job has the lowest patient satisfaction rates compared to other aesthetic procedures. Who should we blame this to? The surgeon? Unrealistic Expectations by the patient? I believe both. Rhinoplasty, is one of the most challenging aesthetic surgeries & thus prone to more botched results. Why does this happen? The older techniques were mostly developed in the 18th century for reducing big noses by removing or trimming the bigger structures. This ‘reductive’ approach fails in many cases. Indiscriminate application of this reductive method is responsible for the over-operated look & for the bad outcomes. The newer techniques are less destructive & focus more on re-balancing the nasal structures rather than merely reducing them. Thus the results are more predictable & refined. Large credit to a successful surgery goes to consultation. It is not a run-off-the-mill surgery. The result define the patient’s looks. Hence the consultation has to be a structured process, getting to know the patient’s desires, analyzing the detailed structure of the nose & artistic analysis of the achievable outcome is extremely necessary. A proper consultation will lead to customized planning with specification of how each internal structure of the nose is to be altered. The surgeon then should conduct the surgery with precision & finesse constantly analyzing each step & its overall effect on the nose. I always recommend to my patients that it’s not for everybody. If you are doing it to please someone or if you think that a perfect nose will get you a better job or improve your prospects, it’s not for you. It is extremely important to analyze objectively the extent of your imperfection & the extent of ‘worry’ associated with it. A good candidate is one who has obvious imperfections which the surgeon & patient can see & feel confident to be able to fix.  We do not make noses to order, we work with the existing nose & try to bring it into better harmony with the face of the owner or to change the nose in line with broad aesthetic principles. I always look at producing a natural looking nose – the Perfect Nose. The key to everything is knowledge. So as you prepare for a Rhinoplasty surgery. A bit of soul searching is in order. Think about why you want to do it?  Identify what is wrong? What you want changed? How sure you are? Are you ready for the new face? What is your tolerance for minor flaws? Because that is a real possibility even with a perfect surgery. The tissue healing process may produce minor imperfections. About 10% Rhinoplasties go for a revision so considered that & know the cost implications should a revision be necessary. Once you are off the surgery table, pain is usually minimal. Under eye swelling blood-stained nasal discharge is common. There will be a splint on top of the nose for a week. Initially the new nose may look swollen & numb. However it will start looking good by the third week. The nose continues to improve till one year. Your doctor will advise you about post-surgery care & massages. Rhinoplasty may be done by plastic surgeons or by ENT surgeons, however, it should be someone who has a ‘feel’ for the job & a passion to do it with precision.
